Name: Your first name or web name. Which ever you prefer. Don't bother with your last name unless you really really want to.

Email Adress: You *must* put down your email address, so we can send you your adoptions. Don't worry, we won't send you junk mail or anything.

Website name: Name of website. You must own a webpage to adopt! I will check to make sure the email addresses match the ones from your site.

Website address: http://etc etc etc. YOUR website address. Not you best friends. Not your Dad's. Not Temple o' Trunks'. If you don't put your own website's address here you won't be sent your adoptions.

First Adoption: Select one from the list. Make sure you've got the right one! The characters are listed in alpabetical order by name. So, if you want eg. Super Saiyajin Goten, he will be listed as Goten (SSJ).

Second Adoption: Same as above. If you only want one adoption leave it as (none). And don't chose the same character twice. (Well okay, you can if you really want to but...)
